I have two questions regarding insurance coverage of mammograms, especially for women over 40 years of age: 1. Why doesn’t the law require insurance coverage for this procedure for individuals who are not part of a group insurance plan? 2. Why does a company make a point of including a special rider that implies this coverage is included when in actual practice it is completely useless?

It seems that since mammograms are particularly encouraged by the American Cancer Society as a preventive measure, insurance companies would be anxious to pay for this rather than very expensive surgical care and treatment for developed cancer.
