Dr. Jack Rizman, 54, a Chicago-area dentist and Arlington Heights resident, died Sunday in Lutheran General Hospital, Park Ridge.He practiced dentistry with his son, Keith, in Wilmette. In addition to his son, other survivors include his wife, Susan; another son, Stuart; a daughter, Kim; his parents; and a brother. Services will be held at noon Wednesday in Congregation Beth Judea, Illinois Highway 83 and Hilltop Road, Long Grove.
