John “Jack” Sullivan, 65, a retired manager with AT&T and a graduate of the University of Notre Dame, died Sundayin Evanston Hospital of cancer. Mr. Sullivan, a native of Evanston, attended St. George High School in Evanston and worked as a caddy for Westmorland Country Club. Survivors include his wife, Maurice Ann; a son John Fitz; his father, Thomas; and a brother. Visitation will be from 3 to 8 p.m. Sunday in William H. Scott Funeral Home, 1100 Greenleaf Ave., Wilmette. Mass will be said at 11 a.m. Monday in St. Athanasius Catholic Church, 1615 Lincoln St., Evanston.
