Daniel R. Bolek, 38, of Addison died March 6 in Grand Junction, Colo. Born in Chicago, Mr. Bolek was a driver for an Addison trucking company. Survivors include his parents, Walter and Loretta Bolek; and two brothers, Don and Dean. Services will be at 9 a.m. Tuesday in Salerno’s Rosedale Chapels, 450 W. Lake St., Roselle, followed by a mass at 10 a.m. in St. Joseph Catholic Church, 330 E. Fullerton Ave., Addison.
