James W. Forward, 73, Indian Head Park, IL, died Sunday, Nov. 5, 2000. Survivors include his wife, Dorothea Maria; sons, John (Beth) of Waterloo, WI and David of Frederick, MD: grandchildren, Jack Stuart, Will Charles and Madelynn Mary; brother, L.
