Frank Jeffrey “Jeff” Polz, 57, died Jan. 5, 2001 of Colon Cancer at his home in Crestwood, KY. Jeff is survived by his wife Joyce; father of Wendie, Beth (Jeff) Blankensop, Laura and Jeffrey; stepfather of Michael Cain and Laura Beth (Jason) Kulma; beloved son of Frank and Marion; brother of John (Carol) and Randy (Lillian); He will be missed also by his two grand daughters Madeline and Abbey Blankensop. Jeff was preceded in death by his grandson Joshua Blankensop. There will be a Memorial Mass for Jeff at 3 p.m. Sat. Jan. 20, at St. Jospeh Church in Libertyville, IL. (121 E. Maple Ave.) following the mass everyone is invited to share fond memories of Jeff and to celebrate his life at the Koenig Center adjacent to the church. In lieu of flowers, memorial donations may be made to the American Cancer Society.
