Stephanie Evasiuk, 82, formerly of Chicago and Phoenix AZ, died March 24, in Hurst TX, she was the beloved wife of the late John Evasiuk; loving mother of Pollyanne (Robert) Szafranski; dear sister of the late Charles (Sophie) Gula, the late Steve (Dorothy) Gula and Anne (the late Jack) Hamel; grandmother of two; greatgrandmother of three; fond aunt of many nieces and nephews. Memorial Service will be held at Nativity of the BVM Ukrainian Church, 8530 W. 131st St., Palos Park, on April 23, at 9 a.m. Arrangements by National Cremation Society.
