Sylvia Denisuk, died Friday, Aug. 27, 2004 in Chandler, AZ at the age of 69. She was born Nov. 1, 1934 in Chicago to Casmir and Mary, nee Sitkowski, Koscielski and has lived in Arizona the last 10 years. Sylvia is survived by her husband, Theodore “Ted” Denisuk; sons, Ted Denisuk (Jean), Phillip Denisuk (Peggy) and John Denisuk; sisters, Delphine Sarnowski, Jean Kornacki, Esther Morgan and Irene Tarte; brother, Daniel Koscielski; grandsons, Nicholas, Alex, Dale and Justin Denisuk. She was preceded in death by a son, Paul, in 1974. Visitation will be 5 to 8 p.m. Tuesday, Aug. 31, 2004 at Bueler Mortuary, 14 W. Hulet Dr. in Chandler. The Funeral Mass will be 10 a.m. Wednesday, Sept. 1, 2004 at St. Steven’s Catholic Church, 24827 S.
