Frank Azzi, formerly of Chicago, died peacefully while in hospice care at Appleton Wisconsin Medical Center on Sunday, June 10, 2007. A memorial service will be held in the chapel at Valley VNA in Neenah, Wis., on June 23, 2007. In lieu of flowers, a memorial is being established in Frank’s name at Valley VNA, 201 E. Bell Street, Neenah, 54956.
