* A story Sunday about vaccination rates in nursing homes quoted the Illinois Department of Public Health’s report that said Symphony Care Network’s 87th Street location had a staff vaccination rate of 2%. A Symphony Care spokesperson disputed that figure and said the staff vaccination rate it reported Aug. 8 to the Centers for Disease Control and Prevention was 57%. A spokeswoman for the IDPH responded that it was a reporting issue between the CDC and Symphony Care.

Additionally, a letter to the editor on Thursday cited the original Sunday story, which has been revised online.
