*A June 6 news story and a June 9 editorial about Chicago police Officer John Ardelean gave inaccurate details about the car crash that led to reckless homicide and aggravated DUI charges against him — charges that were dropped this month. Information in the Tribune about how fast the off-duty officer was driving and what lane he was in came from the original charging document. But a later report from a traffic reconstruction expert hired by the Cook County state’s attorney’s office found that Ardelean was traveling about 40 mph and was in his proper lane at the time of impact.

*A story in Saturday’s Tribune about a new strategic plan for Cook County’s Health & Hospitals System incorrectly described the firm Health Management Associates. It specializes in research and consulting in the health care field.

The Tribune regrets the errors.
