*A June 15 editorial stated that the space shuttle Columbia tragedy was caused by foam insulation hitting the shuttle’s belly. In fact, the foam hit the leading edge of the wing. The editorial also should have noted that astronaut Sally Ride was the first U.S. woman in space, not the first woman in space.

*A Chicago Confidential column by Melissa Harris on the front page of the June 5 Business section incorrectly described Ty Warner as the co-founder of Beanie Babies. He founded the company that makes Beanie Babies.

*A story Sunday about former Gov. Len Small on the Chicago Flashback page gave an incorrect year of his acquittal. It was 1922.
