It`s back! For the sixth straight year, we`re asking you, our readers, to vote for your favorites in the world of entertainment.

The basic rules for Friday`s Readers` Poll-Chicago`s most extensive survey of the public`s entertainment favorites-are simple:

– Only one ballot to a customer. (More than one voter in your household?

Use a blank sheet of paper-or wait for next week`s Friday section when we`ll reprint the ballot; if you`re interested only in a few categories, you can even write your picks on the back of a postcard.)

– No photocopies of the ballot will be accepted.

– While we have no objection to organized get-out-the-vote efforts for your favorite performer or spot, we reserve the right to disallow votes that are obviously mass produced. In other words, if you`re going to stuff the ballot box, think of your Chicago heritage and be creative.

– Half votes and split votes will not be counted.

– You do not need to vote in every category.

– Ballots must be received by 5 p.m. Friday, Jan. 19.

To help you make your picks in three important categories, once again we`re printing checklists of all new movies released in Chicago and all albums and singles that appeared in the Top 10 pop charts last year. You`ll find the checklists in the Friday Guide in the center of this week`s section-along with this year`s Movie Challenge. (You can, of course, vote for a 1989 song that did not make the charts; however, only movies that opened in Chicago in 1989 are eligible.)

All entrants will be eligible for our bonus drawing. This year`s prize is a very special weekend. We`ll be drawing the name of one lucky voter to receive a luxury weekend at the Swiss Grand Hotel, including lodging for two nights in an executive suite, dinner at the Cafe Suisse, breakfast, cocktails in Americus Bar, complimentary valet parking and champagne on arrival.

Complete rules for our bonus drawing are printed below. But remember: To be eligible for our prize drawing, we must receive your entry by 5 p.m. on Jan. 19. Poll results-and our prize winners-will be announced in the Friday section of Feb. 2.

All ballots must include name, hometown and phone number. Fill in this entry blank (or write your ballot picks on a separate sheet of paper;

photocopies will not be accepted) and mail it to: Friday`s Readers` Poll, Chicago Tribune, Box 7437, Chicago, Ill. 60680-7437. (No purchase required.)

To be eligible for the drawing, entries must be received by 5 p.m. Friday, Jan. 19.

The winner will receive a luxury weekend for two at the Swiss Grand Hotel, including lodging for two nights in an executive suite, dinner at the Cafe Suisse, breakfast, cocktails in Americus Bar, complimentary valet parking and champagne on arrival. The prize must be utilized before January, 1991.

All entries must be submitted by U.S. mail. Only one entry per person, and you must enter in your own name. The winner will be announced in the Friday section on Feb. 2.

The Chicago Tribune will not be responsible for entries lost or delayed in delivery and reserves the right to disqualify any mutilated, altered or illegible entry or entries that do not comply with these rules.

The winner agrees to allow the use of his or her name and picture by The Chicago Tribune for publicity purposes and agrees to be responsible for any income-tax liability resulting from the prize.

The contest is subject to all federal, state and local laws, and is void where prohibited by law. Chicago Tribune employees and their families are not eligible.

The winner will be selected in a random drawing of all eligible entries. By entering this contest, all entrants agree that The Chicago Tribune has the sole right to decide all matters or disputes arising from the contest and that The Chicago Tribune`s determination of the winning entry shall be final and binding.

No entries will be returned. All become the property of The Chicago Tribune.
