Representatives of the EMSF Cardiology Association told police that a patient may have stolen a pad of prescription forms from the company and attempted to get a forged prescription filled at a Mt. Prospect pharmacy. Company officials reported the pad stolen after the pharmacy called to verify a prescription for a woman it had never treated, according to police records. The description of the woman matched that of an present patient, police said. The incident is still under investigation.
