John David “Jack” Mathis, 73, of South Barrington, formerly of Libertyville, at rest Oct. 13, 2005, owner of Jack Mathis Advertising, author, coach, pianist and cinema consultant, beloved husband of Dene, nee Hoffman; devoted father of Kane (Meg) Mathis of Seattle, WA and Jana (George Balis) Mathis of Chicago; cherished grandfather of Kaira Mathis; loving son of the the late George and Bernice, nee Bennethum. Memorial Service 1 p.m. Monday, Oct. 17 at The Arlington Heights Evangelical Free Church, 1330 N. Douglas Ave., Arlington Heights. Interment private.
